Seven arrested in Sullivan County welfare fraud cases

LIBERTY – The Sullivan County Welfare Fraud Task Force announced seven unrelated arrests on various charges.
Richard Vogel, 25, of liberty, was arrested for violating the conditions of his parole. After a member of the task force observed him purchasing alcohol in violation of his parole conditions, a joint investigation with state Parole resulted in his arrest. He is currently awaiting a parole hearing.
Philip Jacobs, 29, of Monticello was charged with three felony counts of filing a false instrument for filing and three misdemeanor counts of theft of service. It is alleged that Jacobs, a parolee, stole $143.42 in public-funded transportation by using Medicaid-funded medical transportation for rides to and from his parole meetings in South Fallsburg. He was remanded to the Sullivan County Jail in lieu of $7,500 bail.
Michael Weaver, 47, of Monticello was charged with one felony count of filing a false instrument and one misdemeanor count of theft of services. It is alleged that Weaver, a parolee, stole $12 in public-funded transportation by using Medicaid-funded medical transportation for a ride to the Department of Family Services in Liberty after he was told the day before that he could not use that transportation for this purpose. He was remanded to county jail.
Paulette Williams, 44, of Hurleyville was charged with a felony of filing a false instrument and a misdemeanor of theft of services. She allegedly stole $1,286.26 by using Medicaid-funded transportation for non-medical purposes.
Joseph Barone, 43, of Liberty was charged with a felony of filing a false instrument and a misdemeanor of theft of services for allegedly stealing $72 in Medicaid-funded transportation for the non-medical purpose of getting a ride from his residence to Community Services in Liberty.
Kaitlyn Hoag, 22, of Liberty was charged with the felonies of possession of a forged instrument and offering a false instrument for filing and a misdemeanor of attempted welfare fraud. It is alleged that she submitted a false shelter verification form and a false SNAP (food stamp) recertification form to the Sullivan County Department of Family Services in an attempt to obtain benefits to which she was not entitled. She was remanded in lieu of $2,000 bail.
Lisa Kazamarick, 42, of Liberty, was charged with a felony of possession of a false instrument and misdemeanor of attempted welfare fraud. She allegedly provided the county Department of Family Services with a forged shelter verification form in an attempt to obtain benefits to which she was not entitled.
