CareMount Medical teams with Hudson Valley Hospice for care

CHAPPAQUA –  CareMount Medical, the largest independent multi-specialty medical group in New York State, today announced that it has entered into a Preferred Provider Agreement with Hudson Valley Hospice, a nonprofit community agency that delivers compassionate care to individuals faced with life-limiting illnesses.

“Compassionate hospice and palliative care providers serve as important sources of comfort and relief for patients and loved ones facing serious illness,” said Scott Hayworth, MD, FACOG, President and CEO of CareMount Medical. “We are proud to partner with Hudson Valley Hospice as a preferred provider, which will enable our patients to obtain more streamlined access to higher-quality, more fully integrated care as they enter this delicate portion of their healthcare journeys.”

As part of the agreement, which took effect on January 1, the two organizations have implemented enhanced referral and communications policies to drive better care coordination for CareMount patients receiving palliative and hospice care. By creating better alignment between CareMount and Hudson Valley Hospice, the new venture simplifies and streamlines logistics for patients and their families, freeing them to focus on obtaining the care they need.

CareMount’s Preferred Provider Agreement with Hudson Valley Hospice is part of the medical group’s broader strategy to fully integrate care delivery around its independent physicians. Since launching its Medicare Next Generation Accountable Care Organization (Next Generation ACO) in 2018, CareMount has added preferred providers across the acute inpatient hospital, skilled nursing facility and home care settings. These preferred providers coordinate care for CareMount primary care patients under Medicare and commercial payer risk contracts and partner with CareMount to improve quality and clinical-care outcomes. CareMount Health Solutions, CareMount’s management services organization, runs the only physician-led Next Generation ACO contract in the Tri-State area.

“Hudson Valley Hospice is proud to be working with CareMount and its professionals,” said Michael Kaminski, President and CEO of Hudson Valley Hospice. “This agreement will enhance care coordination, and referral processes with CareMount, allowing us to extend our important hospice and palliative care services to more New York families.”

CareMount Medical employs more than 650 physicians and advanced practice providers in more than 45 specialties. CareMount provides comprehensive medical care to over 665,000 patients throughout the Hudson Valley and Manhattan. In addition, CareMount owns and operates urgent care centers, clinical laboratories, and radiology services, as well as endoscopy suites and infusion suites.
