MONTICELLO – The Sullivan County Office for the Aging is conducting a community needs assessment survey to hear from residents what they believe is important to them or relatives as they age.

The survey is one of several ways in which the office is reaching out. The data gathered from this survey is an essential piece of the process that evaluates available resources and supports and identifies gaps in services experience by older adults and their caregivers.

“Information gathered through this survey will become the foundation for the next planning period that runs from 2020-2024,” said Office for the Aging Director Lise-Anne Deoul. “Ideally, the ‘Four-Year Plan’ captures the voices of our public service area and designates funding in the areas most important to the population served by us, so everyone’s responses are very important to us – and will remain strictly confidential.”

The surveys are due back to the office by August 19.

Anyone can complete the survey at www.surveymonkey.com/r/2LYJWZM, or pick up a copy of the survey at the Office for the Aging (inside the Government Center in Monticello) or a local senior nutrition site.

If you are interested in receiving printed surveys for your group, please contact the Sullivan County Office for the Aging at 845-807-0241.
